The Sky Lakes Collaborative Health Center on the Sky Lakes Medical Center campus is home for two primary care clinics.

 

Sky Lakes Primary Care Clinic

 

Occupying the second floor of the Collaborative Health Center, the Sky Lakes Primary Care Clinic offers patients easier access, on-site Lab and x-ray services, and the Drop-in Clinic for existing patients.

Clinic hours for patients with appointments are 8 a.m. to 5 p.m. Monday-Friday, closed weekends and most national holidays.

 

The Drop-in Clinic is open 9 a.m. to 6 p.m. Monday-Friday, closed weekends and most national holidays.

Cascades East Family Medicine Clinic

 

Cascades East Family Medicine Clinic provides primary patient care on the first floor of the new Sky Lakes Collaborative Health Center.

This clinic and its providers continue to be part of the OHSU Cascades East Family Medicine Residency Program and will operate out of the new building with its spacious patient areas and welcoming environment.

 

The Cascades East Family Medicine Clinic is a patient-centered medical home (PCMH), which facilitates partnerships between individual patients and their personal physicians, and, when appropriate, the patient’s family.
