Sky Lakes Medical Center is a not-for-profit, community-owned, internationally accredited acute-care teaching hospital dedicated to the people it serves. Sky Lakes serves more than 80,000 people in the Klamath and Lake counties in south-central Oregon, and Modoc and Siskiyou counties in northern California. As a 176 bed medical center, Sky Lakes is located in the rural community of Klamath Falls, Oregon, and strives to promote wellness in the community. The pharmacy department provides many services to the hospital and the community, including inpatient, ambulatory care, and outpatient pharmacy services.

 

The Pharmacy Residency Program

 

Sky Lakes Medical Center currently offers an ASHP accredited PGY1 pharmacy residency program. We are looking for motivated applicants who are devoted to their efforts of becoming exemplary practitioners. Residents will get the opportunity to work as part of an interdisciplinary team to provide the patients in the community with evidence-based care. Residents will have many opportunities to learn and grow in a variety of care areas throughout the year, developing clinical confidence and leadership skills. The Sky Lakes PGY1 pharmacy residency program offers a unique learning structure, where all rotations are completed longitudinally. Each rotation will be completed at different points throughout the year, allowing residents to re-visit rotations multiple times and build upon the skills gained on other rotations.

 

Residents will have the opportunity to build a foundation in clinical pharmacy practice that will carry forward to the remainder of their career.

 

We accept two PGY1 residents yearly.

Program Structure

 

Learning Experiences: completed longitudinally

 

  • Pharmacy Orientation (7 weeks at the beginning of residency)
  • Internal Medicine/Surgical Rotation (6 weeks to be completed in 2-week blocks over 10 months)
  • Intensive Care and Progressive Care Rotation (8 weeks to be completed in 2-week blocks over 10 months)
  • Emergency Medicine Rotation (4 weeks to be completed in 2-week blocks over 10 months)
  • Ambulatory Care Rotation (8 weeks to be completed in 2-week blocks over 10 months)
  • Antibiotic Stewardship Rotation (4 weeks to be completed in 2-week blocks over 10 months)
  • Pharmacy Leadership/Management Rotation (2 weeks to be completed in 1 week blocks over 11 months)
  • Medication Safety (longitudinal over 11 months)
  • Oregon Pharmacy Teaching Certificate (longitudinal over 11 months)
  • PGY1 Project (longitudinal over 12 months, including 2 dedicated project weeks)
  • Pharmacy Practice Staffing Rotation10 hour shifts every other weekend, in addition to 1 dedicated staffing week
  • Vacation(2 weeks)
  • Electives(8 weeks, oncology elective rotation offered)

  • Participate in various meetings, including patient safety, P&T committee, and physician meetings
  • Attend OSHP fall and spring seminars, ASHP midyear, and other clinical meetings
  • Facilitate discussions and present material on a weekly basis
  • Publish a quarterly pharmacy newsletter
  • Complete a medication utilization evaluation and formulary review

  • If your application is selected, the first step of the interview process will be completed via WebEx over the phone. The applicant may then be invited for an on-site interview.
    • Initial WebEx interviews are typically scheduled during the third week of January.  On-site interviews are typically scheduled in the second or third week of February.
