Sky Lakes Pharmacy Residency Preceptors

Mychal Amos, PharmD, LSS GB

Mychal is the Director of Pharmacy for Sky Lakes Medical Center.  Mychal strives to uphold pharmacy excellence in our organization while also expanding pharmacy services.  These service expansions include 7 day/10hr pharmacist coverage in the Emergency Department, Ambulatory Care Pharmacist presence at Cascades East, and opening a third retail pharmacy location, Sky Lakes Washburn Pharmacy. The latest expansion will be opening a Specialty Pharmacy in late 2024.

 

Mychal is a native Oregonian and originally from the coastal area but has called the Klamath area home since 1986. He attended Oregon Institute of Technology for undergraduate studies before moving up to the Northwest of Oregon to attend Oregon State/OHSU for his PharmD degree.

 

Outside of the pharmacy, Mychal is happily married and has four children who take up most of his time with their rabbit, hamster, cats, & one dog.  He still finds time to escape by sailing on the Klamath Lake, riding his 1982 Goldwing 1100 with over 130,000 miles, or snowmobiling (weather permitting).  He loves Rural Healthcare and the communities we have the privilege to serve.

Samantha Muir, PharmD, BCPS

Sam is our Residency Program Director.  Locally born and raised in the small town of Merrill, Oregon (just south of Klamath Falls), she began her employment with the medical center as a pharmacy courier while attending Oregon Institute of Technology to obtain a Bachelor of Science in Health Sciences degree. Her goals then took her on to Oregon State University to complete her Doctorate of Pharmacy. She had always intended to move back to the Klamath Basin and serve the community she grew up in.  Sam continued to work at Sky Lakes Medical Center throughout her education and directly after graduation she transitioned to working as an inpatient clinical pharmacist (her goal is to be the longest employee in Sky Lakes history!).

 

In addition to the residency program, Sam is the Retail Pharmacy Manger for our three Sky Lakes retail locations, Sky Lakes Outpatient Pharmacy, Sky Lakes Downtown Pharmacy and Sky Lakes Washburn Pharmacy.

 

Sam’s greatest joy is spending time with family, most of which is chasing children.  Her two boys Cooper and Colt, keep her busy with their many activities (football, ji jitsu and hockey just to name a few).  When she isn’t on the field or at the rink, Sam can be found traveling, sailing, running or kayaking.

Josh Thompson, PharmD, BCPS

Josh, our infectious disease, leadership and co-ambulatory care preceptor, has been working hard to expand our ambulatory care pharmacy services through telehealth. He’s also been busy working on a project implementing a new computer system for our outpatient oncology infusion center.

 

Josh comes from Klamath Falls and was a pharmacy technician at Sky Lakes before attending pharmacy school at OHSU. He returned to Sky Lakes after graduating.

 

Josh spends his free time mountain biking, hiking, golfing, and spending time with his two children

Debra Pederson, PharmD, BCPS

Debra, one of our ICU/PCU preceptors, has been expanding our pharmacy presence within the ICU & PCU by enhancing our daily rounding routines. Her pharmacy interests include infectious disease and critical care with the goal to become board certified in critical care medicine and continue to expand pharmacy involvement in the ICU/PCU. Outside of the hospital, Debra is helping to bridge the gap of active pharmacy leadership in Southern Oregon by becoming an active member of OSHP.

 

Having been born at SLMC and raised close by in Chiloquin, OR; Debra has a special connection with and passion for SLMC and the indigenous Klamath Basin patient population. She attended Oregon State/OHSU for undergraduate studies (Bachelors of Science with minor in chemistry) prior to her graduate PharmD coursework. After a short stint of working in Northern Oregon, Debra quickly jumped on a chance to come back to her roots and work at SLMC.

 

Outside of the medical center, Debra is married to her high-school sweetheart with whom she has two dogs and one cat. They enjoy hiking trails, swimming in Klamath Lake, and floating down our gorgeous rivers. On snowy nights, she also likes to read gothic/ghost story novels.

As the youngest of 10 children, Debra is no stranger to large family functions. She even participates as a “carnie” once a year in the local Klamath County Fair with her family as they make doughboy cinnamon and sugary treats!

Suzy Olin, PharmD, BCPS

Suzy, one of our ICU/PCU preceptors, also doubles as one of our “IT” pharmacists. When she is not working inside the ICU she is actively trying to improve our EMR (Epic) as well as fix any Epic problems that arise.

Originally from Tillamook (yes, cheese town Oregon) she moved to Southeastern Alaska at a young age before moving to Klamath Falls where she has been since middle school. Suzy ventured outside of the Klamath Basin to Oregon State University for both undergraduate and pharmacy school and is a devout Beavers fan.

 

Professionally, her interests lie within the inpatient pharmacy realm, specifically the critical care field and she enjoys being an ICU/PCU preceptor.

 

Away from the hospital, Suzy is a renown cook/baker (her pies are amazing), is married, and has two boys & one Siamese cat. They also have a collection of early Ford Broncos. When she gets a break from family, Suzy likes to dance (ballet, jazz, & modern), participate in outdoorsy activities, sewing/quilting, and as mentioned before…bake/cook some amazing food that the inpatient pharmacy greatly appreciates!

 

Jessica Todd, PharmD, BCPS

Jessica is one of our Medical & Surgical preceptors. An Oregon native, Jessica is originally from Pleasant Hill, a small town located along Hwy 58. She stayed in state for both undergraduate and graduate school at Oregon State/OHSU for her PharmD degree.  She came to Klamath Falls as an APPE student and never left thanks to her amazing coworkers.

 

Professionally, Jessica enjoys all aspects of pharmacy and appreciates our rotating schedule that allows her to work in multiple different areas and units regularly.

 

Jessica is married and together they have three boisterous dogs. She enjoys taking the pups on outdoorsy adventures, traveling, and spending time with her nephews. She has a passion for DIY and whenever she is home, Jessica and her husband work on the never-ending project of renovating their 100+ year-old farmhouse and surrounding property.

Revati Patel, PharmD, BCPS

Revati, one of the medical/surgery preceptors, has been a vital component of the residency program and strives to become involved in performance improvement projects that improve patient safety, pharmacist involvement, and pharmacy practice advancement locally and on a state-wide level.

 

After receiving her Bachelors of Science in Biology from Oregon State University, she continued her studies as a Beaver with a PharmD degree from Oregon State/OHSU. Shortly after graduation, she moved to Klamath Falls and began her career at SLMC.

 

Professionally, Revati has a passion for teaching and ambulatory care. Long term, she would like to transition to an ambulatory care clinic pharmacist role and simultaneously become involved in health policy and management.

 

Outside of the pharmacy, Revati loves to travel, experience new cultures, and try different culinary dishes. Aside from travel, she likes to read and watch films. The most linguistically diverse of our bunch, Revati speaks 3 languages fluently!

Brian Michaels, PharmD, BCPS

Brian, the emergency room (ER) preceptor, expanded services into the ER department after his residency at SLMC. He was born and raised in Cincinnati, Ohio where he attended the University of Cincinnati for both an undergraduate degree in biological sciences followed by a PharmD degree. Brian was a member of the first residency class to graduate the SLMC PGY-1 Residency Program.

 

Professionally, Brian plans to continue expanding pharmacy services in the ER, become ER certified, continue giving presentations to SLMC medical staff and Oregon Institute of Technology’s masters in counseling program, as well as become involved in Oregon pharmaceutical societies to advance the pharmacy profession on a state and national level.

 

Outside of the pharmacy world, Brian is married with two cats, enjoys playing trivia, visiting breweries, playing acoustic guitar, and traveling across the country and globe. Brian is currently attempting to learn the Turkish language.

Jenny Foster, PharmD, BCPS

Jenn, emergency medicine (EM) preceptor, earned an Associate’s degree from the University of Cincinnati as well as her Doctorate of Pharmacy degree in 2010. Before coming to SLMC, Jenn was the EM preceptor at The Christ Hospital in Cincinnati, Ohio and an adjunct facility member at the University Of Cincinnati College Of Pharmacy.

 

Jenn is passionate about being a preceptor for residents and APPE/IPPE students. Becoming board certified in emergency medicine and seeking out teaching opportunities locally, regionally and nationally are essential to her growth as a pharmacist and preceptor. Additionally, one of her top priorities will be to continue expanding pharmacy services in the ED.

 

Outside of her professional career, Jenn spends a lot of time with her dog, Juniper. They love to go for adventures including walks around K Falls and finding hikes throughout the PNW. A few of the components to her happiness and health are traveling to experience other cultures and participating in triathlons. When the winter months roll around, she will be snowboarding, snowshoeing, and cross country skiing!

Chalsea Aiwohi, PharmD, BCPS

Chalsea brings her inpatient experience from several different health systems around the country to SLMC. Chalsea completed a PGY-1 residency at a hospital in Chicago, Illinois before coming to SLMC. Here, she enjoys precepting students and residents in her pharmacy area of choice – internal medicine.

 

Our only Hawaiian native, Chalsea hails from Kalama’ula, Molokai but moved to Oregon for undergraduate studies at Oregon State University before moving to Chicago for pharmacy studies at Midwestern University College of Pharmacy (Chicago).

 

When she is able, Chalsea likes to take her son and her one-eyed dog around Klamath Falls. She also likes to watch movies and likes to travel the country and the globe, especially on surprise trips!

Katie Bullock, PharmD, BCPS

Katie is one of our Medical & Surgical preceptors. Katie enjoys precepting residents and students here at Sky Lakes, and her pharmacy passions include infectious disease and internal medicine. ​

 

Katie grew up in Utah, where she completed her undergraduate studies at the University of Utah, then attended pharmacy school at Roseman University. She moved to Klamath Falls to complete her PGY1 residency here at Sky Lakes, then moved to the Oregon coast for a couple of years before coming back, because she missed Sky Lakes so much! ​

 

Katie lives with her husband on a small farm, where they raise goats and chickens. She just welcomed her first child into the family this past September, and loves spending time with her on her days off. In her free time, she enjoys spending time in the outdoors, hiking and fishing.

Elisabeth Manning, PharmD

 

Elisabeth is one of our Medical & Surgical preceptors. She is currently studying to take the BCPS, and her professional interests include protocol development and mentoring future pharmacy professionals.

 

Elisabeth grew up in Florence, Oregon, and completed her undergraduate studies at Oregon State University. She then earned her PharmD from Oregon State University/Oregon Health & Science University. After finishing her PGY1 Pharmacy Residency at Sky Lakes in 2021, Elisabeth spent three years as a night shift pharmacist at St. Charles Health System in Bend. She then returned to Klamath Falls to rejoin our team.

 

Outside of work, Elisabeth is married and kept busy by her two young children, Kelsey (born in 2022) and Wyatt (born in 2023). Her free time is typically a thrilling mix of watching “Bluey,” munching on Cheerios, and chasing after her energetic kids. She also loves camping in her RV, riding ATVs, and competing in Fantasy Supercross with friends—where she proudly holds the title of “Most Enthusiastic Loser.” Fun fact: Elisabeth is the second oldest of ten siblings.

Austin Pliska, PharmD, BCACP

 

Austin was one of our first pharmacy residents to graduate from our program in 2017-2018. He left to pursue a PGY2 in ambulatory at Legacy Health in Portland, OR. He stayed on with them after residency as a Population Health & Clinic pharmacist. Over 4 years with Legacy Health he helped build & establish ambulatory care pharmacy services in two clinics, as well as help revamp population health pharmacy services. He comes back to us with a wealth of knowledge and a goal of expanding pharmacist-led ambulatory care services here at Sky Lakes Medical Center.

 

Professionally, Austin is passionate about expanding the role of pharmacists within the clinics and practicing at the top of scope. He loves seeing patients and developing relationships with those he provides care for. His focus is diabetes & hypertension management, with plans to expand to other chronic disease states (HF, asthma, COPD, HCV) as the program grows.

 

When Austin isn’t working, he spends time with his wife, baby girl, dog, and two cats. They love to go hiking, bike riding & enjoying time outdoors.
